Job Description

LocumJobs Near Me is looking for an Advanced Clinical Practitioner to support urgent primary care clinics in Stoke-on-trent.

You will assess, diagnose and manage acute presentations with a clear governance framework.

Up to 2 days a week with flexible sessions.

Role Snapshot
• ACP clinics focused on same-day access, assessment and safe onward management.

Key Responsibilities
• Provide advanced clinical assessment for undifferentiated presentations and minor illness.
• Formulate differential diagnoses and implement management plans within agreed scope.
• Prescribe and/or supply medicines in line with qualification and local policy.
• Request and interpret investigations, arranging follow-up and safety-netting.
• Escalate complex cases appropriately and liaise with duty clinicians and services.
• Maintain robust clinical records and contribute to service efficiency and quality.

Working Pattern
• Contracted sessions with potential to extend.
• Up to 2 days per week, flexible clinic times and consistent weekly allocation.
• Predominantly same-day appointments with some planned follow-ups.
• Start dates available to suit onboarding timelines.

Requirements
• Relevant professional registration and ACP-level practice experience in UK primary care.
• Independent Prescriber qualification preferred for autonomous clinics.
• Strong clinical reasoning, risk assessment and escalation decision-making.
• Experience with GP clinical systems and results handling processes.
• Up-to-date safeguarding and mandatory training evidence.
